Code of Conduct

Please print the appropriate code of conducts, sign them and submit them to your child's TEAM MANAGER at the first week of practice. You may also turn forms in at our Concessions stand.  Please indicate the "TEAM weight class" your son plays on. Each Team will have their own folder. No one is permitted to participate in the program without the signed contracts. NO EXCEPTIONS!
